The following forms are available for students to complete and submit to Shafston College.
Please select the appropriate form, complete the details required and submit it to info@shafston.edu. If you have any problems completing the form, please visit us on campus to speak to a support staff member who will be able to assist.
- Course Credit - Credit and Credit Transfer Application Form (60kb, PDF)
This form relates to the Shafston Course Credit Policy and Procedure. If you are enrolling in a VET course at Shafston and would like to have previous study that you have successfully completed recognised at Shafston you must complete and submit this form with your application or before the commencement of your course.
- Course Extension Form (45kb, PDF)
This form relates to students who are currently studying at Shafston who would like to extend their ELICOS course of study.
- Course Variation Form (45kb, PDF)
This form relates to students who are currently studying at Shafston who would like change their current ELICOS course of study. This form is best submitted in person to a support officer who can talk you through your options and if any costs are involved.
- Deferral application Form (47kb, PDF)
This form applies to students who are either yet to commence their course or have commenced their course with Shafston who wish to defer or suspend their course of study and re-commence at a later date. This form relates to the Shafston Deferral Policy which must be read before completing this form. Please be advised you must submit the supporting documentation verifying your compassionate and compelling reasons with this form. Deferral Application Forms received without supporting documentation can not be processed.
- ELICOS Campus Transfer Request (27kb, PDF)
This form applies to students who have enrolled or commenced their study at either the Shafston Brisbane Campus or the Shafston Gold Coast Campus and wish to transfer to study at the other Campus.
- IELTS Book Request (47kb, PDF)
This form applies to students currently enrolled to study an IELTS Preparation Course at Shafston. Please complete the form and ASK your IELTS teacher/s to sign it. Please return to Reception form them to confirm that you are eligible to collect a book.
- Request for Academic Documents - TESOL (51kb, PDF)
This form applies to students who have recently completed or partially completed their Certificate IV in TESOL course at Shafston. All students must complete this form to request any of the following official academic documentation: transcript (statement of attainment or statement of results) testamur and completion letter. This request form is for currently enrolled TESOL students.
- Request for Academic Documents Form (47kb, PDF)
This form applies to students who have recently completed or partially completed their Certificate III in Hospitality, Certificate IV in Business, Diploma of Hospitality or Diploma of Business course at Shafston. All students must complete this form to request any of the following official academic documentation: transcript (statement of attainment or statement of results) testamur and completion letter. This request form is for currently enrolled Hospitality or Business students.
- Request for Reprint of Academic Documents Form (50kb, PDF)
This form applies to any student who has completed or partially completed a Vocational course of study in the past with Shafston. All past students must complete this form to request reprints of any of the following official academic documentation: transcript (statement of attainment or statement of results) testamur and completion letter.
- Request to Finish Early Form (46kb, PDF)
This form applies to ELICOS students only who would like to finish their English course before their scheduled finish date. students who finish their course early will have their remaining fees held in credit for them to re-enrol and use within 12 months. If you are a student Visa holder and you submit this form your CoE and enrolment at Shafston will be ceased and this may affect your student Visa. This form is best submitted in person to a support officer who can talk you through the process and how this may affect you.
- Request to Return to General English (45kb, PDF)
This form applies to any student who is studying an Academic English course at Shafston and would like to transfer back to General English classes.
- RPL Application Form Shafston (65kb, PDF)
This form relates to the Shafston Course Credit Policy and Procedure. If you are enrolling in a VET course at Shafston and believe you have skills, knowledge and competencies held as a result of formal training, work experience and/or life experience that you would like to have assessed as RPL you must complete this form. In completing this form you will be required to gather and present supporting evidence that verify’s your training and experience. An RPL process fee will apply per Unit to commence the RPL assessment process for you. You must complete and submit this form with your application or before the commencement of your course along with all supporting documentation.
- student Change of Address Advice Form (15kb, PDF)
All students must inform the college with 7 days if you change your residential and/or postal address. Please complete and submit this form if you have changed your address recently. Please also submit this form if you have recently changed your email address or phone number.
- student Complaint and Appeal Form for Academic Decisions (159kb, PDF)
This form relates to the Complaints and Appeals Policy and Procedure and this must be read before submitting this form. This form applies to students who have been unsuccessful in coming to an agreement on an academic grievance or verbal appeal they may have. students whose course end date has passed and wish to appeal a NYC or Unsatisfactory assessment result must complete and submit this form.

This form relates to the Complaints and Appeals Policy and Procedure and this must be read before submitting this form. This form applies to students who have been unsuccessful in coming to an agreement on a grievance or verbal appeal they may have. The types of related complaints and appeals this form refers to are listed on it.

This form applies to students who are requesting an official letter to be issued by Shafston.
- student Refund Form (56kb, PDF)
This form relates to the Shafston Refund Policy and Written Agreement and this must be read before submitting this form. This form applies to students who wish to request a refund from Shafston for fees not yet used. Your refund request will be assessed as per Shafston’s Refund Policy and Written Agreement. Please complete this form fully and present all supporting documentation when submitting this form.
- student Transfer to Another Provider Form (50kb, PDF)
This form relates to the Shafston Transfer Policy and this must be read before submitting this form. students who have enrolled to study at Shafston and have been granted a student Visa to study at Shafston must complete this form if they are withdrawing from their course at Shafston and transferring to another provider (College) and require a Release Letter. Your transfer request will be assessed as per Shafston’s Transfer Policy. Please complete this form fully and present all supporting documentation when submitting this form. To be considered for an approved Transfer (Release Letter) you must present a valid offer letter from the provider (College) that you will be transferring too. Your transfer can not be processed without all supporting documentation.
- VET student Withdrawal Form (48kb, PDF)
This form applies to all students enrolled in a VET course at Shafston who wish to withdraw from their course and no longer study at Shafston. If you are a student Visa holder on submission of this form your course and CoE will be cancelled with Shafston and this may affect your student Visa. student Visa holders who are withdrawing and going to study at another college will also need to complete the student Transfer to Another Provider Form if you require a Release Letter to present to your new College. Please complete this form fully and submit it with all other corresponding forms as mentioned on this form. You may wish to submit this form in person to the college if you are not sure how your withdrawal might affect you.
